Jim Vandagrifft is driven to discover innovative ways to improve processes and make positive impact in people’s health and quality of life. As an entrepreneur for 24 years, Jim has led the formation of multiple businesses in various industries.
Jim’s passion for impacting the lives of others continues to keep his eyes on developments in health care with his primary focus on the post-acute continuum and in recent years the social determinates of health. Through Preferred Population Health Management (PPHM), which Jim formed to develop a cloud-based population health software platform (PHL), Jim believes he can not only positively impact those most vulnerable, the elderly and the frail, but help the community-based organizations whose mission it is to serve them. Jim is also Co-CEO leading the development of the first national provider network dedicated to addressing SDOH, Preferred Community Health Partners (PCHP), to contract at the enterprise level with managed care organizations and utilize the aging and disability network to impact care for complex patients by providing Care Transitions and Complex Care Coordination.
Over the past 15 years Jim has worked to develop care delivery models for high risk patients
- Indiana Veterans Home – Care Model utilizing physician extenders and specialist for Pulmonary, Nephrology, Geriatric Psychiatry taking readmission rates from 41% to below 6%
- Preferred Home Healthcare – as an owner/operator established 5 Star rating placing it in the top 5% of all agencies for quality
- Long Term Acute Care Hospital and Skilled Nursing Facility post-acute care models for managing high risk populations across the continuum
With 14 startups under his belt Jim has focused on going where the puck is going to be, as Wayne Gretzky attributes to his success, and is driven to find solutions to the most challenging problems.
Jim holds a Bachelor of Science from Indiana State University in Electronics Technology. In his free time, Jim enjoys spending time with his 8 children and two grandchildren while serving on the boards for numerous non-profit organizations. Jim lives in Indianapolis, Indiana with his wife Nora Vandagrifft, RN CEO of Assured Nursing, Inc.
